A 28 yr old male patient complaints of

CHIEF COMPLAINT: 

A 28 yr old male  Patient who is a farmer by occupation , complaints of fever , body pains  and headache  since 2 days.

HISTORY OF PRESENT ILLNESS:

Pt is apparently asymptomatic 3 days back then he developed fever with chills ,body pains and headache and visited nearby local hospital , he was diagnosed with dengue ,the fever subsided but the platelet count was decreased so  he came to our hospital. 

PAST HISTORY : 

Not a known case of diabetes , hypertension,epilepsy, Tb 

TREATMENT HISTORY: 

No  allergies to any known drugs

PERSONAL HISTROY : 

Pt takes mixed diet 

Appetite - normal 

Bowel and bladder movements - normal

FAMILY HISTORY: 

No relevant family history

GENERAL EXAMINATION: 

Pt is conscious coherent and cooperative and examined in a well lit room 

Pallor  -No 

Icterus  - No

Cyanosis -no

Clubbing  - no 

Pedal edema  - no 

Lymphadenopathy -no 

VITALS: 

Temperature - afebrile

Blood pressure - 130 / 90mm Hg

Respiratory rate - 18 cycles per minute

Pulse rate - 76 beats per minute 

SYSTEMIC EXAMINATION : 

CVS : S1 S2 heard .        

No murmers heard.

No thrill palpable 

RESPIRATORY SYSTEM: 

Bilateral air entry is normal

Trachea Position is in central

No dyspnea 

No wheezing sounds 

CNS : 

Pt is conscious 

Speech is normal . No neurological deficits.

PROVISIONAL DIAGNOSIS : 

Dengue NS1 antigen positive 

INVESTIGATIONS : 








TREATMENT : 

Tab  pantop 40 mg  OD

Tab Dolo 650 mg BD

Inj OPTINEURON 1 amp
