CHIEF COMPLAINT :  56 yr old male patient complaints of SOB and pain over left inframammary area since 10 days and cough since 1 week. HISTORY OF PRESENT ILLNESS:  Pt was apparently asymptomatic  4 yrs back then he developed fever, chills , cough ,sob and pain in the left inframammary area and he consulted a doctor and  took medicine and symptoms are subsided  Then now again he developed same symptoms 10 days back and cough since 1 week Sob was gradually progressing and he had orthopnea. CHIEF COMPLAINT:  A 28 yr old male  Patient who is a farmer by occupation , complaints of fever , body pains  and headache  since 2 days. HISTORY OF PRESENT ILLNESS: Pt is apparently asymptomatic 3 days back then he developed fever with chills ,body pains and headache and visited nearby local hospital , he was diagnosed with dengue ,the fever subsided but the platelet count was decreased so  he came to our hospital.  PAST HISTORY :  Not a known case of diabetes , hypertension,epilepsy, Tb  TREATMENT HISTORY:  No  allergies to any known drugs PERSONAL HISTROY :  Pt takes mixed diet  Appetite - normal  Bowel and bladder
