This is an online e log book to discuss our patient de-identified health data shared after taking his / her / guardians signed informed consent.  Here we discuss our individual patients problems through series of inputs from available global online community of experts with an aim to solve those patients clinical problem with collective current best evident based input. A 54 yr male patient came to the casualty with the chief complaint of loss of appetite , nausea , decreased urine output since 20 days . History of present illness:  Patient was apparently well 20 months back, then he developed pedal edema 18 months ago , he went to nakrekal hospital and was given NSAIDS . Then on medication the swelling was subsided. And 8 months back he again developed pedal edema with loss of appetite and went to hyd hospital , then came to our hospital. PAST HISTORY:  Patient has hypertension .  Not a known case of Tb , Epilepsy, Asthma , and Diabetes mellitus. TREATMENT HISTORY:  FAMILY HISTORY:  N

A 62 year old female presented with complaints of pedal edema, shortness of breath, and decreased urine output since 4 days . HISTORY OF PRESENTING ILLNESS:   Patient was apparently assymptomatic 11 months ago, then she developed lower back pain with early fatiguability. She went to a hospital in Khammam  and she was prescribed NSAIDS for 3 months.  Then she then developed loss of appetite, early fatiguability, and swelling of hers arms and legs 7 months ago.
